3. Who is your treating Doctor?
4. What were your Symptoms in the beginning? What are your Symptoms today?
5. Do the symptoms change during the week?
6. How does look a the good time when u feel well?
7. Menstrual cycle, how long? pain? how much blood flow? DO the Symtomps change during the cycle.
8. Do u have pain While intercourse?
9. Are you in a current relationship? Or are you having intercourse at this time?
